Forms
For your convenience, you may print and complete several forms to expedite your check-in process on your initial visit. Please bring the completed forms with you on your initial visit or fax them to Barrington Rehabilitation at (847) 381-6311.
Medical History (required): Please print and complete the Medical History Form.
Patient Information (For Unaccompanied Minors Only): Please print, complete and sign the Patient Information Form if you are unable to attend the initial appointment with your minor teenage son or daughter.
Patient Diagnostic Questionnaires (if applicable): Please print and complete the appropriate questionnaire as it pertains to your diagnosis and bring it with you on your initial visit. The questionnaire will assist the therapist in developing your plan of treatment and therapy goals.
